RENTAL AGREEMENT
Facility
The Community Rooms can accommodate an audience of up to three hundred (300) people seated in rows or up to two-hundred and fifty (250) people seated banquet style. The Community Room can be subdivided into three (3) rooms. It is unlawful to exceed the maximum capacity limits. GC activities/programs have priority over any private rentals. All private rentals are first come, first serve. Facility rental does not include admission to the GC. Proof of residency is required for renters to receive member/resident rate, and the renter must be present for the entire event.
Catering/Kitchen Use
Renters will use their own caterers. All on-site caterers will have certain responsibilities including but not limited to providing proof of insurance and a credit card number to be charged in the event of damages. A contract for the use of the caterer’s warmers kitchen in the Maple room is also required. It is the responsibility of the caterer/renter to provide all dished, utensils, decorations, and linens. Youth ages 16 and under are not permitted in the kitchen, and youths 16 through 18 years of age must be accompanied by an adult. The renter will be required to complete an exit survey of the rental facility, staff, and caterer. Caterer must contact the Office Manager 30 days prior to the event.
Alcohol
Alcohol consumption is limited to the confines of the Community Room and must comply with State of Ohio liquor laws. Proof of special event insurance/host liquor liability must be provided (normally through your homeowners’ policy). The City of Fairview Park must be added as an additional insured for $1 million for the day of your event, and a permit must be signed. A minimum of one (1) Fairview Park police officer must be present during your event. Based on the number in attendance, additional security may be needed and can be filled with auxiliary police officers. Security rates, determined by the Police Department to be $25 per hour, 3 hour minimum, with the officer required to stay ½ hour beyond end of the function. Contact information for police security may be obtained through the Office Manager.
A. If liquor is sold for cash, or if included in the ticket cost:
- A liquor permit is required from the State of Ohio. This is the responsibility of the renter (if no concessionaire is involved). Renters will call 614.644.3155 to secure the permit application.
- A liquor permit is secured, then the City of Fairview Park requires that the liquor permit holder also secure liquor liability insurance at a limit not less than $1,000,000. A certificate of insurance, as evidence that such insurance coverage has been secured, must be presented to the city 30 days prior to the event.
- Police Officer- Arrangements are made in advance through the Fairview Park Recreation and Community Center. Only a Fairview Park police officer may be used at your event. The fee is $25.00 per hour, minimum of 3 hours. The officer must stay one-half hour beyond the ending time of the event to ensure that all guests exit promptly.
B. In order to serve alcohol (no sale) at your event, the Fairview Park Recreation and Community Center requires the following:
- Insurance- You must contact your insurance company and ask them to add the City of Fairview Park as an Additional insured for $1,000,000 to your homeowners insurance for the day of your event. Insurance must cover Host Liquor Liability. The Office Manager must be in receipt of this insurance a minimum of 30 days prior to the event. Please be sure that insurance reads “The City of Fairview Park”, not The Fairview Park Recreation and Community Center, and use 21225 Lorain Rd. Fairview Park, OH 44126, as the address.
- The city of Fairview Park reserves the right to require police officer presence at any event. See item 1 above.
C. General Requirements
- Minors- It is the responsibility of the person signing this contract to ensure that minors will not be served.
- Alcohol is permitted inside the rented room only and is not to be taken outside of the Community Rooms. Should these conditions not be met, alcohol will not be permitted to be served. If damages are incurred, your credit card will be charged accordingly.
Rental Hours
The rental hours of the Community Rooms follow the hours of operation of the Gemini Center. Rental hours can be extended until midnight on Friday and Saturday.
Set-up & Cleaning
Gemini Center staff will set up tables and chairs prior to the rental time. Set-ups and tear-downs of personal effects such as linens, centerpieces, etc, are the responsibility of the renter. Renter is requested to leave the room in the condition in which you found it, wiping down tables & chairs, and bagging all trash. Cleaning supplies are provided. All personal property must be removed. No decorations may be attached to the walls, windows, doors, tables, chairs or light fixtures, and the use of tape, push pins, rice, confetti, “silly string”, glitter and the like is prohibited. Smoking or open flames are not permitted. If necessary, damages will be assessed, and the credit card on file will be charged accordingly. All renters must check out with a building supervisor before leaving the building. Failure to do so will result in a $25.00 fee. There will be a $30.00 per hour extended hour cleaning charge for rentals that are scheduled after building close time. See Extra Item sheet.
Parking
Rentals that occur during operating hours with parties of 50 or more will be required to park in the back parking lot located off W213th Street until spaces are full; parking will then proceed to the front parking lot. Parking will not be permitted in the library or offices next to the center. Cars will be towed.
Smoking
The Gemini Center is a non-smoking facility. Smoking will only be permitted 20 feet from the front (main) entrance doors. All guests must dispose butts in the designated can outside. A $150 fee will be charged to renter(s) for smoking in prohibited areas.
Deposits/Cancellations
One-half (1/2) of the room rental fee is due at contract signing, and a credit card (MC/Visa/Discover) must be provided as a security deposit. Should damages occur; the credit card on file will be charged. The balance of the room rental fee is due thirty (30) days prior to the event. Reservations are not considered final or confirmed until full payment of all fees is received by the Gemini Center. Cancellations for sectional rental of the Community Rooms must be done thirty (30) days prior to the event and for the entire Community Room sixty (60) days prior to the event.
Management reserves the right to cancel a rental if facility rental policies are violated.
