All guests must be accompanied by a member. Proof of residence is required for a pass to be sold at the resident rate.
Guest per member ratio: 18 and over (Maximum: 4 guests)
17 and under (Maximum: 1 guest)
You must be at least 10 years of age to sign in a guest.
Please Note:
* *A family (up to 4) membership is defined as 2 adults and 2 children under the age of 18 with the same address or 1 adult and up to 3 children under the age of 18 with the same address. Children 2 and under are free and must be listed on application form. A maximum of 6 members permitted. Exceptions to be approved by the Director of Recreation.
* **Full-time College Student rate is also available for $25 during Winter Break (Dec. 1 through Jan.31) and $50 during Summer Break (May 1-September 1). Students must show proof of enrollment.
* Individual and youth memberships (those ages 10-17) must be completed by parent or legal guardian.
* Two current forms of residency or corporate verification must accompany application for new or renewed memberships.
* A copy of birth certificate or valid driver’s license must accompany any age verifications for new memberships.
* All memberships are non-refundable and non-transferable
* Memberships can be purchased by cash, credit card (Visa/MasterCard/Discover) or a personal check made payable to the Fairview Park Recreation Department.
* A membership package may be upgraded (add college age student, upgrade from individual to family, etc.) at any time, however, the expiration date of the original membership becomes effective for all members; fees will not be prorated.
* Caregiver and Personal Aid passes can be obtained if applicable. Please contact the recreation office for more details.
BUILDING RULES AND REGULATIONS
Must be 10 yrs of age to enter building without an adult.
Children 7 and under must have direct adult supervision.
All youths under the age of 13 must exit the building at 8:00 pm, unless accompanied by a parent/guardian or are in a registered program.
Must be 15 yrs. of age to utilize the Fitness Center. Ages 13 and 14 may use the Fitness Center with restrictions.
Proper and valid membership card and/or valid identification is required to gain access to the center.
Food and beverage are permitted in the snack bar or designated areas only.
Profanity, inappropriate gestures and public displays of affection will be prohibited.
Spitting and gum chewing are not permitted in the facility.
Changes in the Recreation Department schedule may occur without notice. Pool and gym schedules are available at the front desk
No pets are allowed except those assisting persons with a disability and/or if a special program is scheduled.
Proper dress required. No intimidating or vulgar attire permitted.
Loitering will not be permitted in the hallways or other areas of the recreation center.
The use of cell phones/camera phones are prohibited in the restrooms, locker rooms, fitness, and pool areas. Improper phone use will result in disciplinary action.
Only scuff-resistant shoes are permitted in the activity areas of the building. Muddy or dirty shoes are not permitted. Dark soled running shoes, turf shoes, street shoes and any types of shoes which mark the floors, especially our gym, track and aerobics floor are absolutely prohibited. Shoes are required to be worn at all times while in the facility (except in locker rooms, sauna and pool area) or at the discretion of a management.
The Gemini Center is not responsible for lost or stolen articles. It is strongly recommended that you do not bring any valuables into the Center. We encourage that you place all belongings in a locker and secure them with a personal lock (lock needs to be removed by closing time or it will be removed by management.)
All guests are required to be signed in and accompanied by a member of the recreation center.
All visitors must sign in and out at the reception desk and present picture identification.
Discipline Policy
Foul language, inappropriate behavior/verbal, continuous horseplay, not following building policies, etc.
Step 1: Warning
Step 2: 3 day suspension
Step 3: 1 week suspension
Step 4: 1 month suspension
Step 5: 6 month suspension
Step 6: 1 year suspension
VIOLENCE physical contact, verbal harassment (extreme), theft
The police will be notified in the event of physical violence.
Step 1: 1 week
Step 2: 1 month
Step 3: 3 months
Step 4: 6 months
Step 5: 1 year
ALL INCIDENTS WILL BE EVALUATED BY THE DIRECTOR. THE FINAL DISCIPLINARY DECISIONS WILL INCLUDE ALL PREVIOUS OFFENSES.
IF A MEMBER OR VISITOR IS SUSPENDED; SUSPENSION INCLUDES ALL RECREATION ACTIVITY (LEAGUE PLAY/PRACTICES, PROGRAMS, ETC.)
